Note to applicants: The following application form must be completed and delivered to the Superintendent’s Office at Putnam County High School, along with the required documents listed below, by August 1, 2008. The application form and the documents should be placed in an envelope that is clearly marked for delivery to the “Alumni Association Scholarship Committee”.

I have attached the following documents to this application form:
1. One-page statement of my activities and achievements at Putnam County High School;
2. One-page statement of my career goals and objectives; and
3. One-page statement of my involvement in college, community, and civic activities.
I understand that the two scholarships available from the UHS/PCHS All-School Alumni Association will be awarded during the annual all-school alumni reunion on Saturday afternoon, August 30, and I will be present for the awards ceremony, unless excused by the alumni association chairman.
I also understand that the scholarship funds are to be used for the 2008-09 academic year, and that I will not be eligible for the funds if I do not enroll and attend classes at the college I have named above for the 2008-09 academic year.
